What does ' monkey malfunction ' mean and if a chimp malfunctions what do you do ?

I think from the context I saw it used, it referred to the need for error condition checking and defensive programming as a GUI was easy to use, so easy a train monkey could use it but sometimes even a trained monkey makes a mistake and pushes the wrong on.
